To join the JAFSCD Shareholder Consortium, please fill out this form. If you have unique circumstances, use the "Additional Info" box to explain. You will receive a confirmation email once you click "Process My Share Purchase."
It is recommended that you have the following ready before filling out the form:
The full name, affiliation, and email address of your chosen representative to the Shareholder Consortium. Otherwise, note in "Additional Comments" that the name of a rep is forthcoming.
Your choice of payment (either PayPal or offline, to request an invoice)
Note regarding payment methods: By default, payment will be made using PayPal (which you can pay via a PayPal account or any credit card). Use the offline payment method if you prefer to be invoiced. Use the Additional Info field to indicate if the invoice should be addressed to someone else in your organization.
Note that no organization or program that wishes to become a JAFSCD Shareholder will be turned away for lack of ability to purchase a share. Contact Duncan Hilchey to discuss options.
